Program Assessment
Shape Up US offers 2 types of assessments for the
Hip Hop Healthy Heart Program for Children™
1. Educational Assessment
With the completion of each module, students are required to complete a written test to assess their comprehension of the topic. We offer a multiple choice/ true-false test. Depending on the aptitude and abilities of their students, teachers can also opt to include essay questions to add language arts and critical thinking to the mix. Upon passing each module test, students will receive a certificate of completion and will be eligible for additional awards and prizes.

2. Health Assessment
To make sure that the positive lifestyle lessons taught in the Hip Hop Healthy Heart Program for Children™ are truly making a difference, Shape Up US has partnered with HeartSmartKids to track each student’s health progress.
What is HeartSmartKids? HeartSmartKids is a web-based system for quickly creating a summary of a child’s cardiovascular (heart health) risks. HeartSmartKids is used in healthy living programs, medical offices, and community health events. HeartSmartKids quickly gathers and presents information related to a child’s health. This allows providers, community health workers, and program administrators to see risks more clearly and thus provide better information to parents.
What information is collected by the system? Height, weight, birth date, and gender. In addition HeartSmartKids will gather a special set of information for the Hip Hop Healthy Heart Program for Children™ that includes family history of cardiovascular risks and other lifestyle issues. This lifestyle information may include eating habits such as how much soda or what type of milk a child drinks. HeartSmartKids does not collect names, addresses, social security numbers, or phone numbers.
How long does it take to administer the evaluation for each student? 5 minutes.
Who will administer the evaluation? The teacher, or school nurse, classroom assistant, community health worker or volunteers.
How often will the HeartSmartKids evaluation be administered? Once at the beginning of the program to create a baseline for each student, and once at the end of the program to track the student’s health, nutrition and fitness progress.
